Frequently Asked Questions (FAQ)
Where is Dr. Streicher’s office?
Dr. Streicher’s office is located in Northwestern Medicine’s Lavin Family Pavilion, 259 East Erie (on the corner of Fairbanks and Erie) on the 24th floor. Until the new Center opens in early 2017, Dr. Streicher will temporarily see her patients in the Northwestern Medical Group Fertility and Reproductive Medicine practice, also located on the 24th floor of the Lavin Family Pavilion.
Will Dr. Streicher’s NMG office take insurance?
Yes, but because visits will be extended, patients who wish to join will pay an annual fee to receive expanded and personalized services, as described on here. With the exception of the Personal Care annual fee, your insurance will be billed as usual for the care provided during your office visits, and you will still be responsible for co-pays and/or co-insurance amounts as required by your health plan. Our staff will be available to assist you if any reimbursement questions or concerns arise during the billing process.
How do I enroll to be a patient in Dr. Streicher’s Personal Care practice?
Please complete and return the Membership Application and Agreement form found here and return it per the instructions to Dr. Streicher’s patient concierge. The concierge will contact you to confirm enrollment and provide further instructions regarding your medical records and other items. If you need more information, please contact the concierge at
312 472-0502 or firstname.lastname@example.org,
Is Dr. Streicher going to continue to give community talks?
Yes, even more. Dr. Streicher’s passion is to educate and connect with women, so to that end, she will be conducting a series of events and talks throughout the Chicago area. Please fill out the contact form to receive invitations and updates.
Is the annual fee considered a qualified health care expense that can be paid from a Health Savings Account (HSA) or Flexible Spending Account (FSA) and/or is the fee tax deductible?
Unfortunately, the annual fee is not considered a qualified expense that can be paid or reimbursed from an HSA or FSA, because it is not considered payment for direct care or services. Generally, the annual fee is not tax deductible either.
